About Patient-centered Medical Home (PCMH):

The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction –while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement. Recognized practices submit an annual report summarizing ongoing quality improvement and population health activities to sustain their recognition. Sustained recognition provides external validation that a practice using advanced population health functions to continuously improve outcomes for their patients. Nationally, approximately 13,000 practices (with 67,000 clinicians) are recognized by NCQA. West Virginia has 104 recognized practices, including six WVU Medicine primary care practices. Six additional WVU Medicine practices are participating with NCQA in a transformation process and will seek recognition in the coming months.
