Transition of Care Clinic

The Transition of Care Clinic (TCC) is a hospital discharge clinic that provides patients with a bridge from the WVU Hospital discharge to their home, when additional assistance is often needed. The clinic is available for all patients seen by WVU Internal Medicine and for those patients who do not have a primary care doctor. The clinic provides comprehensive care to patients during the visit and will help transition their care to a primary care office through the collaboration of our nurse practitioners, physicians, pharmacist, social worker, nurse, and medical assistant.
