Families, NICU staff, and community supporters are welcome to order a t-shirt based on this year's "World Travelers" theme.  Cost is $10 per shirt.  Please print form below and mail payment request to:

Make checks payable to “West Virginia University” and place NICU reunion on the memo line.  Shirts are available for pick-up at the June 2^nd NICU Reunion.  Order ahead to make sure you get your favorite color and size!  It is also possible to purchase t-shirts on-site at the NICU Reunion while supplies last.
